The nature of the response to both hGH and AOD9604 is not clearly understood. Acromegaly is characterized by an excessive amount of articular cartilage in joints caused by excess GH secretion [25]. Metabolic turned their attention to human studies and performed six clinical trials on 925 people between 2001 and 2007. Not one of these trials showed that AOD9604 caused any weight loss or changes in body composition (3). Osteoarthritis (OA) is a degenerative joint disease that results from articular cartilage loss induced by complex interactions of genetic, metabolic, biochemical, and biomechanical factors with secondary components of inflammation [1]. OA is the most common arthritis and a major medical problem in people aged 65 years and older [2]. Non-steroidal anti-inflammatory drugs (NSAIDs), physical therapy, exercises, corticosteroids, and hyaluronic acid injections have all been used for the conservative treatment of OA. The role of NSAIDs in OA management is controversial because of its adverse side effects, as well as side effects on the cartilage [3].
We identified 34 patients who met eligibility criteria based on the chart review. Two subjects were excluded because they were found to have diabetes requiring insulin therapy, 1 was excluded because of hypothyroidism, and 13 were excluded because they did not undergo gastric bypass surgery or did not wish to participate. A final study sample of 18 individuals (15 women) was enrolled into the protocol. We had 3 individuals who did not complete the 6‐month follow‐up visit; the sample that completed both visits consisted of 15 individuals (12 women). No subjects were excluded because of peri‐operative complications. Table 1 displays the characteristics of the study sample at baseline and 6 months after surgery. From baseline to 6 months after surgery, subjects had a mean decrease of 27% in body mass index (P<0.0001). There were significant reductions in mean arterial blood pressure (P=0.004) and heart rate (P<0.001) after surgery. Only 2 out of 18 subjects were on any class of anti‐hypertensive medications at the pre‐op visit before gastric bypass surgery. At the 6‐month visit, anti‐hypertensive medication was discontinued for one of these subjects, and continued at the same dose for the other subject. The mean±SD volume of saline infusion pre‐bypass was 2.6±0.4 L and post‐bypass was 2.3±0.3 L.

The size and duration of the metabolic responses to AOD9604 in the β3-AR KO animals was different from that observed in the control wild-type mice. The response was more rapid, shorter in duration, and greater in peak response. This may be because the KO animals are more acutely sensitive to lipolytic agents, a compensation for the ablation of the major lipolytic receptor.

In this paper, we investigated whether the changes observed inβ 3-AR RNA expression in vitro also occur in an in vivo model. The in vivo model used was the obese (ob/ob) mouse model of obesity that has repressed levels of β3-ARs, which in part contributes to reduced lipolytic sensitivity (12). Lean C57BL/6J mice were used as a control. Following a 14-d chronic administration with AOD9604 or hGH, adipose tissue weights were measured, and β3-AR mRNA expression was determined. The decrease in weight of adipose tissue depots in the ob/ob mice was associated with increasedβ 3-AR expression. Further studies inβ 3-AR knock-out (β3-KO) mice showed that the presence of the β3-AR is necessary to mediate the chronic effectiveness of hGH and AOD9604 with regards to weight loss and fat mass reduction.
