Ipamorelin is very similar to the growth hormone releasing peptides (GHRPs) GHRP 2 and GHRP 6 in that it mimics ghrelin (the hunger hormone) and targets a specific HGH pulse. It is well established that hGH is a lipolytic hormone (15), but the exact mechanisms used are still unclear. In this paper we present data that suggest that hGH and its lipolytic fragment (AOD9604) induce their chronic in vivo actions on lipolysis in part by modulating the expression of theβ 3-AR. Human GH has been shown to affect the in vivo expression and function of β-ARs in vivo in sheep (16). Data presented in this paper indicate that chronic administration of hGH influences expression of the β3-AR in adipose tissue in the ob/ob mouse. In brown adipose tissue (BAT), these compounds also increase expression of β3-AR expression in the lean C57BL/6J mouse. The increase in expression induced by chronic hGH or AOD9604 treatment correlated with the decrease in adipose tissue mass. We therefore hypothesize that treatment with either hGH or AOD9604 enhances β3-AR expression, which has been observed in murine 3T3-F442A and human SK-N-MC cells in vitro (11).

But gene-therapy experiments have also resulted in patient deaths. Resting plasma concentrations (mean±SEM) of mature ANP and Nt‐proANP were 14±2 pg/mL and 4±0.6 pg/mL at baseline. After the commencement of active treatment, 78.7% of subjects experienced at least one AE, with the incidence ranging from 75.6% to 83.2% across all treatment groups (83.2% placebo group; 75.6% 0.25 mg AOD9604 group). There was a high incidence of infections and infestations (46.8%, mainly nasopharyngitis, 17.1%), nervous system disorders (30.1%, mainly headache, 25.9%), musculo-skeletal and connective tissue disorders (25.5%, mainly back pain, 8.2%), and gastrointestinal disorders (22.9%, mainly diarrhea, 7.8%). Although the percentage of subjects experiencing AEs in these body systems is higher than in the run-in period there was no obvious pattern with respect to treatment received, suggesting that the increase was likely due to the longer period of assessment in the active treatment phase.

AOD9604, a synthetic fragment of hGH consisting of the amino acid residues 177–191 with the addition of a tyrosine residue to the N-terminus was prepared with solid-phase synthesis procedure and purified with reverse-phase HPLC methodology in our laboratories (13). In summary, we found that weight loss in obese individuals undergoing gastric bypass surgery is associated with higher natriuretic peptide concentrations across a range of loading conditions.
