A study research coordinator screened charts for eligibility from a pool of patients who were referred to the weight center of the Massachusetts General Hospital for Roux‐en‐Y gastric bypass surgery. There were no changes in laboratory parameters or vital signs in any treatment group. There were no clinically significant abnormalities in vital signs, safety tests, or ECGs during the studies. The objective of the 3rd study (METAOD003) was to assess the safety, tolerability and pharmacodynamics of single oral doses of AOD9604 in healthy, clinically obese males. 17 subjects (n = 15 completed the study), age 35 to 54 years, with a BMI ≥ 35 kg/m2 (range 35 to 56 kg/m2) subsequently received 3 increasing doses of AOD9604 (9, 27 and 54 mg) or placebo. Each dose administration was separated by a 2-week wash-out period.

The original GRF (1-29) has a half-life of about 30 minutes. Half-life means the time within which half of the hormone administered will be destroyed within the body. This short half-life is due to the fact that the compound is highly unstable and breaks down soon. To increase its stability and to make it last longer, it was modified by adding 4 amino acids in its structure. This gave it the name Modified GRF (1-29) or Mod GRF 1-29. It was originally invented by DatBtrue. The portion of the molecule that actually stimulates the growth hormone secretion is found in the chain of 29 amino acids, so it is named GRF (1-29). This chemical also produces slow-wave sleep.

Three of the submissions did not support the proposal highlighting the impact the change in scheduling would have on product currently on the market, industry, pharmacists and consumers. One submission, referring to the NEJM article, believed that a lack of information about the study means that it cannot be relied upon as there is not a meaningful assessment of the results.

The ability of AOD9604 and hGH to increase the level ofβ 3-AR RNA expression in obese mice to a level that is comparable to those in lean mice is an exciting finding. However, it must also be considered that both hGH and AOD9604 may influence the expression of other members of the adrenergic pathway, such as the β1-ARs, hormonesensitive lipase, and signaling proteins, which are all expressed in adipose tissue and associated with lipolysis. The importance of the change inβ 3-AR expression with AOD9604 and hGH in humans is not established and will depend on the use of potent and selectiveβ 3-AR agonists that are active at the human receptor.

Raising GH levels with any peptide would accelerate fat loss obviously but if looking at strictly fat loss HGH frag 176-191 and AOD9604 (frag 177-191) are the fat loss peps. They are the part of the HGH amino acid sequence that initiates lipolysis. I personally like Hexarelin dosed 3x a day, it is the strongest ghrp and doesn't have any effect on hunger so it works great for fat loss and dieting for me.

AOD is a peptide 15 amino acids long which mimics a small portion of the growth hormone that has the fat reducing effects (increases fat metabolism). The hGH has two different binding regions, site 1 and site 2, which bind in a sequential manner to two different regions of the receptor. Only if this trimer of one hGH molecule and two receptors is formed, does the subsequent signal transduction pathway become initiated [27, 28]. The hexadecapeptide AOD9604 consists only of amino acids 177-191 of hGH with an additional tyrosine residue at the N-terminus. The binding site 1 of the hGH, which is located in the fourth helix [27], is partially overlapping with the sequence of AOD9604. However, binding site 2 of hGH is completely missing in AOD9604. Therefore, it was hypothesized that AOD9604 is unable to induce dimerization and thereby activation of the receptor. This has been confirmed in previous in vitro experiments. In the first dose-escalating study (METAOD001) 15 healthy male subjects received 3 single dosages of AOD9604 and placebo as single dosages each separated by a 7-day washout period (range 25 to 400 µg/kg bodyweight; single IV infusion doses over 20 minutes). One subject terminated the study due to personal reasons, 14 subjects completed the study. In total twenty-nine AEs were reported by twelve subjects during the study. No SAEs occurred during this study. The most common AEs reported during the study were headache (6 times). The remainder were related to fatigue (4), hypoglycemia unspecified (3), dizziness (3), nasopharyngitis (2), cough (2) and lethargy, tonsillitis, abdominal pain unspecified, application site reaction unspecified, sore throat unspecified, injection site bruising, rhinitis seasonal, anorexia, injection site pain, all with an incidence of 1. None of the AEs were of severe intensity. The majority of AEs were mild in intensity with possible relationship to study treatment, equally distributed between the various concentrations of AOD9604 and placebo treatment. The adverse event profile was similar following administration of all treatments.

Ghrelin and GH secretagogues, including GH-releasing peptide (GHRP)-6, stimulate food intake and adiposity. Because insulin modulates the hypothalamic response to GH secretagogues and acts synergistically with ghrelin on lipogenesis in vitro, we analyzed whether insulin plays a role in the metabolic effects of GHRP-6 in vivo. Streptozotocin-induced diabetic rats received saline, GHRP-6, insulin, or insulin plus GHRP-6 once daily for 8 wk. Rats receiving saline suffered hyperglycemia, hyperphagia, polydipsia, and weight loss. Insulin, but not GHRP-6, improved these parameters (P < 0.001 for all), as well as the diabetes-induced increase in hypothalamic mRNA levels of neuropeptide Y and agouti-related peptide and decrease in proopiomelanocortin. Cocaine amphetamine-related transcript mRNA levels were also reduced in diabetic rats, with GHRP-6 inducing a further decrease (P < 0.03) and insulin an increase. Diabetic rats receiving insulin plus GHRP-6 gained more weight and had increased epididymal fat mass and serum leptin levels compared with all other groups (P < 0.001). In epididymal adipose tissue, diabetic rats injected with saline had smaller adipocytes (P < 0.001), decreased fatty acid synthase (FAS; P < 0.001), and glucose transporter-4 (P < 0.001) and increased hormone sensitive lipase (P < 0.001) and proliferator-activated receptor-gamma mRNA levels (P < 0.01). Insulin normalized these parameters to control values. GHRP-6 treatment increased FAS and glucose transporter-4 gene expression and potentiated insulin's effect on epididymal fat mass, adipocyte size (P < 0.001), FAS (P < 0.001), and glucose transporter-4 (P < 0.05). In conclusion, GHRP-6 and insulin exert an additive effect on weight gain and visceral fat mass accrual in diabetic rats, indicating that some of GHRP-6's metabolic effects depend on the insulin/glucose status.
